Job summary The urgent and intermediate care team are pleased to be offering these posts to join our Care at Home Team, providing patient centred care to individuals in the community, alongside community nurses and allied health professionals.We are looking for passionate and experienced HCSWs to join our Pembrokeshire Team to support patients to stay in their own home.Interviews to be held on 17/12/24 Main duties of the job In this varied and exciting role, you will be providing personal care to support patients in their own home.
This can be for patients waiting for long term care, reablement and rehabilitation, continuing health care, or as part of their end of life / palliative care plan.The post holder will be required to deliver a high standard of person centred care in accordance with Care and Treatment Plans as directed by the registered nurse.
Excellent interpersonal and communication skills are essential as is the ability to work autonomously and with colleagues.You will be working in the Community but have a contractual base at either South Pembrokeshire Hospital or Withybush General Hospital.
The service operates 7 days a week between 0730 - 2200 and staff shifts are within these hours.You will be working in patients' homes across the county and the ability to travel to locations in a timely manner is essentialThe ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply About us Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties.

We do this in partnership with our three local authorities and public, private and third sector colleagues, including our volunteers, through:Four main hospitals: Bronglais General in Aberystwyth, Glangwili General in Carmarthen, Prince Philip in Llanelli and Withybush General in Haverfordwest.Five community hospitals: Amman Valley Hospital and Llandovery Hospital in Carmarthenshire, Tregaron Hospital in Ceredigion and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire.Two integrated care centres (Aberaeron and Cardigan, Ceredigion).Community facilities, including: 48 General Practices (GP surgeries), 49 Dental Practices, 98 Community Pharmacies (chemists), 44 General Ophthalmic Practices (including eye health and low vision services), 38 sites providing mental health and learning disability services, care within your own homes.Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.

Person Specification Qualifications and Knowledge Essential NVQ Level 3 in Health and Social Care or working towards with equivalent level of experience.
