Main duties of the job Our Practice Nurses are supported by our Lead Nurse and deliver practice nursing services within the boundaries of their scope of practice to the practice population.They will focus on clinical delivery such as chronic disease management to include Asthma/COPD, Diabetes, health promotion , a range of treatment room skills including vaccinations, immunisations, family planning and women's health.To work collaboratively with the whole Practice team ensuring that the care delivered to the Practice population is relevant, responsive, of the highest standard and is clinically effective and efficient.

Job description Job responsibilities Routine immunisations and vaccinations given under a Patient Group Direction (with additional paediatric vaccination certification an advantage) Cervical Screening Contraceptive pill checks / sexual health Undertake long term condition reviews with appropriate training (eg Asthma/COPD, Diabetes, CKD, Hypertension) To deliver opportunistic health promotion where appropriate Offer a holistic approach to travel health by providing comprehensive advice and risk assessments to patients prior to travel including illnesses, vaccinations and medicines, safe sex, food hygiene, sun protection Participate in local initiatives to enhance service delivery and patient care Person Specification Qualifications Essential Registered Nurse with current NMC registration and active PIN Health Care Management/Leadership Qualification Chronic Disease Management Qualifications Competence to undertake long term condition reviews Evidence of appropriate knowledge base Personal Attributes Essential Ability to listen, empathise with people and provide person centred support in a non-judgemental way Able to support people in a way that inspires trust and confidence, motivating others to reach their potential Ability to maintain effective working relationships and to promote collaborative practice with all colleagues Able to provide leadership and finish work tasks Demonstrates personal accountability, emotional resilience and works well under pressure Ability to organise, plan and prioritise on own initiative, including when under pressure and meeting deadlines High level of written and oral communication skills Knowledge of, and ability to work to, policies and procedures, including confidentiality, safeguarding, information governance and health and safety Experience Essential Experience of working in a healthcare setting supervising staff Portfolio of CPD with evidence of ability to maintain professional registration Desirable Experience of working in primary care Experience of GMS contract and QOF requirements
