Job Description About the Role We are seeking an experienced and dedicated Education, Health and Care Plan (EHCP) Co-Ordinator to support the implementation of the Special Educational Needs (SEN) Reform programme in line with the Children and Families Act 2014 , associated regulations, and the SEND Code of Practice 2014 .
You will work collaboratively with children, young people, families, and multi-agency teams to create, implement, and review EHCPs, ensuring that outcomes are person-centred and aligned with statutory guidelines.
Key Responsibilities Assessment and Evidence Gathering: Lead assessments, gather relevant evidence, and triage families to appropriate services.
Person-Centred Planning: Facilitate multi-agency planning meetings, draft outcomes-focused EHCPs, and ensure compliance with legislative requirements.
Review and Monitoring: Regularly review the outcomes of EHCPs and support schools with pupils who have SEND to achieve their potential.
Dispute Resolution: Address disputes and support tribunal preparations if necessary.
Support and Guidance: Provide advice on personal budgets, appropriate educational placements, and the Equality Act's requirements for SEND pupils.
