Rehabilitation Team Coordinator

Job summary Aunique opportunity has arisen within Adult Critical Care at LTHT for a Band 6RN to join the Rehabilitation team and also deliver interventions on thecritical care wards.
This is a 12 month secondment to cover maternity leave,and huge opportunity to develop the service patients on critical care receive.Therole requires working within a small team to co-ordinate the day to day runningof the service.
This involves delegating tasks within the team, coordinatingand communicating effectively with the wider multi-disciplinary team to setachievable individual goals and delivering rehabilitation interventions.Thesuccessful candidate will be supervised by the Team leader of the Critical Carerehab team, The successful candidatewill be supported to further enhance their assessment, intervention and complexclinical assessment and rehabilitation skills with patients who are admitted tocritical care.
support are also available.Theteam is committed to providing best evidence based interventions andrehabilitation within this clinical setting An understanding and enthusiasm ofrehabilitation is paramount to provide an effective patient-centred approach tothe role as well as clinical experience working at Band 6 level.
Main duties of the job Thesuccessful candidate will work within a small team planning and deliveringpatient centred and holistic approach to rehabilitation and recovery; improvingthe physiological and psychological outcomes for patients following anadmission to critical care.Theywill be responsible for co-ordinate the day to day running of the service.
Thisinvolves delegating tasks within the team, coordinating and communicatingeffectively with the wider multi-disciplinary team to set achievable individualgoals and delivering rehabilitation interventions.
