Job summary This is a Fixed Term Contract opportunity for up to 1 year to cover a period of Maternity Leave.
A secondment would be considered for any internal candidates.This is anexciting opportunity to join the Community Forensic Team at one of the Country's most respected providers of secure mental health services.We are lookingfor a highly motivated Support Time Recovery Worker to join the team based at Easby House,Langdon Hospital, in Dawlish.
You will support individuals in their recovery journeythrough secure care and into the community.
You will support the caseload toaccess opportunities to develop their skills, improve their health, wellbeing,and quality of life whilst promoting their reintegration into the community.

Main duties of the job Youwill become an important part of the team, working to ensure that every patientreceives high quality care.
You will provide support to both inpatient and community based service user, and have an activeinvolvement in the delivery of a high quality, safe and compassionate mentalhealth service.Youwill be supporting the Care Co-ordinators (CCOs) with the assessment and careplanning process, including active participation in a range of clinicalmeetings.You will aidwithinitial assessments which may include; gathering information to support thisprocess, support with assessment documentation, contributing to the formulationprocess and any other related tasks under the supervision (directly andindirectly) of CCO.You will support the CCOs in the care planning process wheredirected.
This may include; writing care plans for specific aspects of care,gathering information to support this process, support with careplanningreviews and any other related tasks.Use of specific standardised assessmenttools including carers assessments and physical health assessments (undersupervision of a CCO).
This will include the use of relevant frameworks and documentation forspecific clinical areas.You will contribute to care co-ordination and care managementresponsibilities including making applications for funding panel for enablingsupport social care packages, the monitoring and review of a caseload including activecase reviews (under the supervision of a CCO).

Person Specification Qualifications Essential *Knowledge of mental health and/or social care practice acquired through NVQ Level 3, PLUS training to diploma level or equivalent, equivalent knowledge obtained through short courses OR relevant work based experience.
*Must already hold or be willing to obtain the Care Certificate within the first 3 months of employment.
*GCSE in Maths and English (Grade C or above) OR Level 2 Key Skills / Functional Skills in Maths and English.
Desirable *Additional formal and experiential training in working with children & adults who have complex needs.
*Foundation Degree in Health and Social Care Experience Essential *At least 2 years experience of working with people with complex co-morbidities and vulnerabilities mental health, learning disability, ADHD, autism, offending behaviours, substance misuse, homelessness, debt etc.
*At least 1 years experience of working with people who are hard to engage through their vulnerability, who are challenging in presentation OR employ risk behaviours including self-harm *Working within a community setting in either the statutory or voluntary sector *Ability to assess and manage risk *Lone working with people who use services Desirable *Experience of working in a multi-disciplinary and multi-agency environment Knowledge and Skills Essential *Knowledge of a range of Mental Health presentations and medications *An understanding of physical health problems which can impact on a persons mental health and monitoring of the same.
*Knowledge of safeguarding principles and appropriate action to take.
*Basic IT skills *Understanding of Recovery principles in Mental Health Desirable *Awareness of legal restraints impacting on the rehabilitation of individuals on the Community Forensic Team caseload *An understanding of Care Coordination/ Management process *Awareness of the Code of conduct for Health Care Support Workers /Adult Social Care Workers in England Additional Criteria Essential *Ability to accept and use supervision appropriately and effectively.
*To demonstrate empathy, compassion and patience, towards people with mental health difficulties.
*To remain professional at all times including challenging situations.
