Job summary St Helens North Primary Care Network are looking to recruit to a newAdvanced Nurse Practitioner who is a caring, compassionate and experiencednurse who, acting within their professional boundaries, will provide care forthe presenting patient from initial history taking, clinical assessment,diagnosis, treatment and evaluation of care.
They will demonstrate safe,competent clinical decision-making and expert care, including assessment anddiagnostic skills, for patients within general practice.The successful applicant will work within our friendly Primary CareNetwork multidisciplinary healthcare team alongside another PCN ANP in the samerole and 2 Pro-active Frailty ANPs, and in partnership with the six practicesthat make up the Primary Care Network.We are unable to accept application from applicants who require Visa Sponsorship.No recruitment agences please.
Main duties of the job Within our PCN, we have recently created an Urgent Hub model where allsix PCN practices will be able to book in to your acute hub clinic forsame-day patients.
Using your clinical expertise, the primary focus of the roleis to work as an autonomous practitioner providing high-quality and timelypatient-centred care to assess patients who present with a broad range ofsituations and conditions.The post holder will beresponsible for the assessment, diagnosis, treatment, referral, and dischargeof a diverse range of patients across the age continuum with acute illnesses; independentlywithin their level of competence but with GP support when needed.The second part of the role willbe time spent in individual practices, seeing their patients only to providecare for both acute issues and long-standing conditions depending onpractitioner competencies.
Depending on a successful applicantsbackground/clinical interests there is the possibility to work with practicesto offer specific clinics/services in this time instead.Other aspects of the role will beworking alongside/covering other ANP colleague in the team who covers carehome, working with the Clinical Director and Clinical Lead to help develop theurgent hub service and also other services for the PCN, offering education andmentoring to more junior nursing colleagues or students in the PCN.The successful candidate will have an advanced practicequalification & be a non-medical prescriber.

Job description Job responsibilities The post holder will be an autonomous practitioner who will be expected to work independently in the role of assessing and managing patients with both acute and long term conditions, with the main focus on seeing patients in the PCN Acute Hub but also working within practices.The ANP will have a key role in supporting the work of the PCN Clinical Lead in transforming the care of patients requiring same day assessment to better support practices to address some of the aspects of Modern General Practice Access described in the Delivery Plan for Recovering Access to Primary Care.The Advanced Practitioner role within the PCN will provide comprehensive healthcare sensitive to the needs of the population, including advanced clinical assessments, history taking, diagnosis, plan and deliver treatment, prescribe and evaluate care.
Working as part of a multidisciplinary team and across the primary, secondary and social care interfaces in St Helens.The post holder will provide highly specialised health assessments and input as an independent and autonomous practitioner developing and leading relationships with members of the multi-professional team allowing for consultation and collaboration in avariety of health care settings.The post holder will lead in developing, delivering and evaluating high quality patient centred interventions in conjunction with the associated clinical teams to ensure this is incorporated within both the overarching PCN Services and the St Helens Local Care Organisations objectives.Act as an ambassador and role model supporting the aims and vision of the PCN.Act as a resource to all staff in delivery of clinical care to patients.Please see Job Description for full details.
Person Specification Experience Essential Substantial post registration experience with leadership experience Proven experience of working as an independent practitioner In-depth knowledge of recent NHS legislation, relevant to community.
Experience of leading, planning and completing audit/research.
Experience of teaching and mentoring of staff across the multi-disciplinary team.
Desirable Evidence of applying research in practice.
Post Graduate Qualification Change management experience.
