Job summary An exciting opportunity has arisen for anenthusiastic, well-motivated and flexible Band 3 Health Care Support Worker tojoin our Urgent Community Response and Virtual Ward Services across SouthTyneside.This role is based with the Recovery at Hometeam based at Clarendon.
The team consists of an extensive MDT whichincludes nurse practitioners, occupational therapists, physiotherapists, healthcare assistants, therapy assistant practitioners and administrative staff.
Theteam also works closely alongside partner agencies including those from thelocal and voluntary sector.Full time or part time hours will be considered for this post.Please note the Trust are not able to offer Skilled Worker Sponsorship for this role as it does not meet the UKVI threshold requirements.

Main duties of the job This role would involve supporting people intheir own homes who are either acutely unwell or have recently been dischargedfrom hospital and require enhanced intervention and short-term support fortheir physical and functional health needs to remain at home.
The service covers 24 hours, 7 days per week,it is fast paced, and you would have the opportunity to see a wide range ofpatients who present with a variety of conditions.
The service is committed to providing a soundenvironment for education and training to support the successful candidatewithin this role.

Job description Job responsibilities PLEASE REFER TO THE ATTACHED JOB DESCRIPTION FOR FULL DUTIES OF THE ROLE.Tosupport the Recovery at Home clinical staff service to provide care to peoplein their own homes to prevent admission/ readmission to hospital over a 24-hourperiod.Towork with the team undertaking roles set out in the patient management care plan.Toassist the clinical team in undertaking clinical observations to supportdiagnosis and treatment.Toperform diagnostic tests.Toschedule own calls as directed to meet needs of the service.Maintainaccurate written and electronic information systems, supporting data input ontoIT systems.Onoccasion there may be a requirement to support administration colleagues withtelephone calls and transferring information onto the designated clinicalsystem.
Person Specification Qualifications Essential NVQ level 3 in Health and Social care / or on the pathway to achieve / or demonstrable equivalent skills and experience.
Level 2 Maths and English (GCSE A-C or equivalent).
Successful completion of the Care certificate.
Experience Essential Experience of working in NHS or healthcare environment.
Experience of working collaboratively with a wide range of staff.
Desirable Experience in an acute care setting.
Multi- disciplinary/ multi-agency working Community nursing experience.
Previous experience of working within a team.
Skills and Knowledge Essential Good communication skills both verbally and written.
Ability to recognise and report any clinical issues.
Ability to maintain confidentiality.
Treats people with dignity and respect.
Works to agreed standards & within protocols under supervision.
Flexible working over 7-day period.
Team worker.
Effective Communication skills.
Able to work without close supervision.
Accepts own limitations.
Willing to undertake further training to support role development.
Desirable IT skills.
Experience of recording information in a clinical IT system.
Experience of performing diagnostic tests eg; Venepuncture, ECGs.
Physical Skills Essential Ability to move and handle safely without causing harm to themselves or others.
Car Driver with full UK diving licence.
Desirable Have been trained in moving and handling procedures.
