Main duties of the job As a Practice Nurse, you will play a key role in providing essential patient care within our local community.
Your responsibilities will include: Delivering immunisations and vaccinations to patients of all ages.
Managing chronic disease clinics, such as asthma, diabetes, and hypertension.
Undertaking cervical screening and providing contraceptive advice.
Promoting health and wellbeing through lifestyle advice and patient education.Youll work collaboratively with our multidisciplinary team to ensure the best outcomes for our diverse patient community while enjoying opportunities for professional growth and development.

To beinstrumental in the support of the Infection Prevention Control (IPC) lead andto work to the IPC requirements by providing audits, training, support andevidence of compliance to the practice managerb.
To assessthe needs of patients ensuring the provision of healthcare is appropriate,incorporating evidence-based practicec.
Toimplement and evaluate individual treatment plans for chronic disease patientsthat promote health and wellbeingd.
To beinstrumental in smear recall programmee.
Toidentify, manage and support patients at risk of developing long-termconditions, preventing adverse effects to patients healthf.As required, to provide routine nursing careto patients in accordance with clinical based evidence, NICE and the NSFg.
To reviewpatient medications to enhance complianceh.
Toco-ordinate the provision of travel medicine servicesi.To request pathology services and processpathology results as requiredj.To oversee the provision of chronic diseaseclinics, delivering patient care as necessary, referring patients tosecondary/specialist care as requiredk.
Tomaintain accurate clinical records in conjunction with extant legislationl.To ensure SNOMED CT codes are used effectivelyby all members of the nursing teamm.
To ensurethe appropriate maintenance and use of chronic disease registersn.
To ensurecompliance with the practice chaperone policyo.
To assistGPs with minor surgery when requiredp.
To supportthe team in dealing with clinical emergenciesq.
Toimplement vaccination programmes for adults and childrenr.To support patients in the use of theirprescribed medicines or over-the-counter medicines (within own scope ofpractice)s.To liaise with external services/agencies toensure the patient is supported appropriately (vulnerable patients etc.
)t.To delegate clinical responsibilitiesappropriately (ensuring safe practice and the task is within the scope ofpractice of the individual)u.
To supportthe clinical team with all safeguarding matters, in accordance with local andnational policiesv.To deliver opportunistic health promotionwhere appropriate Person Specification Qualifications Essential Essential: Registered Nurse (Nursing and Midwifery Council) Desirable Desirable: Mentor or teaching qualification Post graduate diploma or degree (Chronic Disease Management) Family planning qualification Experience Essential Essential: Experience of working in a primary care environment Experience of working autonomously Experience of working as a practice nurse or community nurse Experience of infection prevention and control measures Clinical knowledge in the following: ECGs Venepuncture New patient medicals Immunisations (routine, childhood and travel) Womens health (cervical cytology, contraception, etc.)
