Job description Job responsibilities We are currently recruiting for aqualified Clinical Pharmacist to work at The Lakes Medical Practice.The role will be an integral partof day-to-day Surgery functioning with opportunities for patient facingroles to clinically assess and treat patients using their expert knowledge ofmedicines alongside scope for specialist clinics and prescribing.The responsibilities will includeconducting Structured Medications Review and Medicines Optimisation to tacklepolypharmacy, audit and review of medicines through NHS-led programmes such asLow Priority Prescribing and ICS/PCN driven cost saving switching.For an appropriate candidate,there will be potential scope for professional development in evolving skillsin independent prescribing.
This will include being responsible for long termchronic conditions including asthma/COPD, diabetes, cardiovascular medicine andcommunity based anticoagulant monitoring and finally managing structuredmediation reviews and medication management in the two Nursing Homes we providemedical care for.
This will take the format of both telephone and face to faceconsulting Person Specification Qualifications Essential Qualified pharmacist PgCert Pharmacist Independent Prescribing Qualification or equivalent Desirable Experience of working in General Practice, working with General Practice clinical systems Experience Essential Demonstrable commitment to Continuous Professional Development; Flexible approach to work; Effective verbal and written communication skills; Effective time management skills to balance work and education requirements.
