Background – Do you have a passion for working with elderly and frail patients?Are you committed to improving outcomes and ensuring patients like these can be treated in their place of residence and kept out of hospital beds?Do you want to manage your own day and have the autonomy to develop your own treatment plans?If any or all of these are yes, think about enquiring about this role further.An existing specialism in Frailty is not required for this role, but studying for the diploma will be a pre-requisite of the role, with this paid for by the provider.Salary – £10,000 per 4-hour sessionLocation – Scarborough (occasionally covering Ryedale and Whitby areas)The provider – Not-for-profit organisation owned by all of York's GP practicesDelivering urgent care services in collaboration with the hospitals and GP practices in York, Scarborough, Whitby, Malton and SelbyAlso provide community diagnostic centres with the aim of shifting services out of hospitals and into the community'We are part of the system, and we are here to stay' Your role – 2-4 sessions per week but full-time may be availableA typical day will be a maximum of five visits with most of these in and around ScarboroughCompleting clinical assessments and elements of comprehensive geriatric assessments (with support from York Hospital-based Geriatrician)Developing personalised treatment plans alongside the MDTTypically caring for patients for around five days, with a soft limit of fourteen daysTypical working hours will be 8.30-5.00 with 30-minute lunch but can be flexible on start and finish timesDaily morning huddle and 3pm board meeting to review the day's cases and plan tomorrow'sYou must be qualified as a General Practitioner and registered with the GMC and UK Performers List The benefits – 6 weeks' annual leaveStudy leaveBMA model contractNHS pension
