Job summary Our Community team work in patients' homes, linking closely with the GP practices in Mid Hampshire, covering week days only at present.
Our community teams are based in Winchester City, Colden Common and Andover.
This post will cover the Winchester and the surrounding rural areas providing rehabilitation for both integrated intermediate care and long term conditions.We are a busy, friendly, cohesive team, who would welcome the opportunity to work with a new member of staff willing to contribute to the on-going development of the services we provide.
Previous experience working in a care or rehabilitation setting is highly desirable.The work in our team very varied and you will have opportunity to develop your skills in a variety of ways.
You will receive on the job training in line with our competency framework and with support from our qualified staff, as well as having opportunities to attend relevant courses.
You will also be given training and support to gain The Care Certificate.
You will need to have good time management and IT skills.
We are looking for a motivated individual who is keen to work as part of a team for the best outcome for our patients.Applicants must have access to a vehicle for work and possession of a full driving license is essential.
Main duties of the job To implement specific treatment programmes as guided by an Occupational Therapist or Physiotherapist to patients in the community or ward based who present with a variety of health conditions.
