Job summary Rural Rother PCN is looking for a Salaried GP to join our PCN team.
This post is available through the Additional roles reimbursement scheme funding.
The role is for a GP who is newly qualified in the last 2 year and has not held a substantive post in another practice.
We are looking to fill 12 session per week with 2 part-time GPs.
You will be supporting our 7 Rural GP Practices, Fairfield, Oldwood, Martins Oak, Sedlescombe and Westfield, Northiam, Rye Medical, Rye Ferry Road, with a mixture of face to face and remote working.
This role is initially until 31st March 2025, with a view to extending to a permeant post if the funding is secured beyond that date.
Main duties of the job Face to face consultationsCovering clinical management of patients in surgery as appropriate including initiating examinations, reviewing results, making referrals etc.Undertaking clinical activities to support practices to achieve QOF other Practice and PCN quality targets.Patient home visits and nursing home visitsAdministration dealing with patient queries, patient test results, referral letters, NHS/Private tasks, emails About us Our PCN GP partners are very a friendly and supportive group of GPs.
We booster a number of GP trainers in our practices who are happy to support and mentor newly Qualified GPs.
Our PCN utilises our PCN staff on a collaborative based model, we employ Physios, Clinical Pharmacists, Pharmacy techs and Social prescribers who work both in practice and remotely in our central office in Battle.
We also have 2 ANP who support the management of online consultations and care homes.All our Practices are generally held in high regard by our patient, the ICB and other local healthcare providers.
They have active and supportive Patient participation groups, and they regularly achieve high rankings in national GP survey.
All of our practices have CQC ratings as good.Our practices work well together, sharing peer group learning via our GP Quality and Innovation group, Practices managers weekly support forum and shared PLT learning activities.Six out of seven of our Practices are dispensing Practices and depending on how the role develops there maybe opportunities to step into Practice partnerships in the future Job description Job responsibilities To undertake a variety of duties including surgery consultations,telephone consultations and queries, visiting patients at home, checking andsigning repeat prescriptions and dealing with queries, paperwork, reports andcorrespondence in a timely fashion.Making professional, autonomous decisions in relation to presentingproblems, whether self-referred or referred from other health care workerswithin the organisation.Assessing the health care needs of patients with undifferentiated andundiagnosed problems.Screening patients for disease risk factors and early signs of illness.In consultation with patients and in line with current Practice diseasemanagement protocols, developing care plans for health.Providing counselling and health education.Admitting or discharging patients to and from the caseload and referringto other care providers as appropriate.Recording clear and contemporaneous consultation notes to agreedstandards.Collecting data for audit purposes and complying with QOF requirements.Delivering enhanced services.Compiling and issuing computer-generated acute and repeat prescriptions(avoiding hand-written prescriptions whenever possible).Prescribing in accordance with the Practice prescribing formulary (orgenerically) whenever this is clinically appropriate.In general the post-holder will be expected to undertake all the normalduties and responsibilities associated with a GP working within primary care.
Person Specification Qualifications Essential Fully qualified GP with GMC registration 2 years of CCT and not yet held a substantive position post qualification.
Where appropriate will have undertake Annual appraisal and revalidation be on NHSE performers list Applicants must have current UK Professional registration and the right to work in the UK
