Working alongside the multidisciplinary team the H@H team provide high quality palliative and end of life care to patients wishing to remain in their own home.
We are inviting applications from candidates with a passion for palliative care and end of life care, looking for career development in this area.
Candidates will need to work flexibly within the H@H service providing social, emotional, psychological and wider well-being care to our patients and their families.
