JOB TITLE: Rehabilitation Worker

REPORTS TO: Care Services Manager

PAY: £12 PER HOUR

LOCATION: ENFIELD

HOURS: 36 PER WEEK

OVERALL PURPOSE

To enable tenants to manage their tenancies and move towards greater independence, by providing a flexible and individually tailored rehabilitation programme.

To work effectively as part of a team, providing interventions for tenants to redevelop and maintain lost skills and abilities.

KEY ROLES

Supporting Tenants:

1. Have a Comprehensive understanding of the initial care needs assessment process and the required methods of providing ongoing assessment of needs.

2. Devise rehabilitation support plans in conjunction with tenants, setting out objectives, interventions, time frames and outcomes.

3. Whenever possible, assist tenants to write their own support plans and agree review dates and monitoring mechanisms.

4. Provide pro-active daily support and monthly key-working sessions, which focuses on supporting the tenant to achieve the goals as set out in their support plan.

5. Devise risk assessments/risk management plans, based on the needs and risks as identified in the initial assessment and identify new risks as they develop.

6. Have an in-depth knowledge of the ongoing needs of each tenant and identify new issues as they arise

7. Encourage and assist with promoting the independence of tenants by recognizing and developing existing skills, in ways that take account of their individual abilities, anxieties, preferences and health needs.

8. Work in a manner that demonstrates respect for the tenant’s rights and wishes.

9. Provide in-depth feedback on a regular basis to colleagues and managers, of the progress and needs of tenants.

10. Record comprehensive daily notes that clearly demonstrates monitoring and identifying how each tenant responds to the interventions as set out in their support plan.

11. Ensure tenants’ health care needs are identified and maintain a log of when and how these were addressed.

12. Support tenants to develop and maintain relationships with family and friends outside of their home.

13. Maintain regular communication and good relationships with relatives, advocates and other professionals who support the tenants and keep a record of all contact.

14. Support tenants out in the community and on holidays abroad.

Policies and Legislation:

1. Become familiar with and follow all policies and procedures of Paradise Independent Living (PIL), standards of conduct and guidelines for staff.

2. Promote the philosophy of the company in all aspects when working with colleagues and tenants.

3. Take personal responsibility, and support PIL, to meet the requirements of legislation including: Health & Safety at Work Act, Fire Regulations, Confidentiality, Food Hygiene, COSHH, Environmental Health, Equal Opportunities and CQC regulations

4. Identify and address issue, if staff are working outside PIL’s policy framework and legal guidance and report back to the manager or directors, any concerns

5. Support PIL quality standards and help provide evidence of how these are being met. Complete and maintain all necessary up to date records and documentation

6. Undertake any relevant vaccinations as required by legislation and/or guidelines for the business

Individual and Team Work:

1. Support the directors and promote the work of PIL, in a positive light to people inside and outside of the company.

2. Promote and maintain good working relationships between team members.

3. Actively support a consistent team approach to meet the individual needs of tenants.

4. Identify items in and outside the houses that requires repair or replacement and reporting this to the Service Manager.

5. Complete relevant documentation accurately and to the required standard when on shift

6. Be available to work shifts on a rota, according to the needs of the tenants. Providing support on any day of the week, including bank holidays and sleep-ins.

7. Undertake other duties as required by the Care Service Manager that will support the service.
