Rough Sleeper Navigator

Are you passionate about tackling homelessness and supporting some of the most vulnerable people in society? My client is seeking a dedicated Rough Sleeper Navigator to provide high-quality homeless prevention, advice, and support services to rough sleepers in the borough. This role is integral in helping individuals move from homelessness to permanent housing, working closely with outreach services and Supported Accommodation Projects.

Key Responsibilities:
·Provide comprehensive support and advice to rough sleepers, including assessment of their housing needs and developing Personal Housing Plans.
·Work closely with outreach services like London Street Rescue and No Second Night Out to engage rough sleepers and offer solutions.
·Ensure each rough sleeper receives tailored support, including help with income maximization, navigating benefits, and attending medical or DWP appointments.
·Assess rough sleepers' eligibility for housing assistance in line with relevant legislation, including the Housing Act 1996 and the Homelessness Act 2002.
·Maintain accurate case records and data, ensuring that performance is reported as required.
·Reconnect rough sleepers to their local area if necessary, and make referrals to supported accommodation services for vulnerable individuals.
·Collaborate with other agencies to ensure rough sleepers receive holistic support, including mental health, substance misuse, and employment services.

What We're Looking For:
·Proven experience in homelessness prevention, supporting vulnerable individuals, and providing housing-related advice. Knowledge of rough sleeping issues and relevant legislation is essential.
·Strong communication, case management, and problem-solving skills. Ability to manage complex cases and work collaboratively with internal teams and external partners.
·A thorough understanding of housing legislation, welfare benefits, and the specific challenges faced by rough sleepers, including immigration issues.
·A proactive and empathetic approach to supporting individuals, with a commitment to helping people move towards sustainable housing.
