Social Worker - Adults Community Team (West)

Location: Southend-on-Sea

Salary: £36,648 - £42,403 per annum (pro-rata for 30 hours per week)

Key Responsibilities:

Conduct assessments of individuals' care and support needs.
Develop and implement care plans in collaboration with service users and their families.
Provide practical support and guidance to individuals in the community.
Work closely with healthcare professionals and external agencies to coordinate holistic care.
Maintain accurate and up-to-date records, ensuring compliance with statutory requirements.About You:

Qualified Social Worker registered with Social Work England.
Experience in adult social care, with a focus on community-based services.
Strong knowledge of relevant legislation and policies in adult social work.
Excellent communication skills and the ability to build positive relationships.
