Job Title: Social Care Officer

Location: Newham

Rate: £22 Umbrella P/H

We are seeking a motivated and compassionate Social Care Officer to join our dynamic team in Newham. In this vital role, you will provide high-quality, responsive, and safe assessment, support planning, and review services to adults and their families/carers. Key Responsibilities

Conduct assessments, support planning, and reviews to meet the needs of adult customers and their carers, ensuring compliance with national and local standards.
Deliver personalized services that promote independence, choice, and risk management.
Assist customers and carers with supported self-assessments, identifying innovative solutions to meet their needs.
Manage an allocated caseload effectively, providing a professional social care service.
Contribute to safeguarding adults by assessing risks and developing multi-agency safeguarding plans.
Act as a lead in meetings with customers, their families, and external agencies to ensure inclusive and effective support.
Maintain up-to-date, accurate, and clear records in line with statutory requirements.
Collaborate with internal and external partners to strengthen support networks and improve service delivery.What We're Looking For

Experience working in a social care or related setting.
A commitment to delivering high standards of practice, including person-centered and outcome-focused services.
Strong knowledge of safeguarding, risk management, and the Mental Capacity Act 2005.
Excellent communication, organizational, and interpersonal skills.
Ability to manage a varied caseload and work collaboratively within a multidisciplinary team.
