Support Worker
Liberty Centre is currently recruiting for a number of Support Workers and Driver Support Workers.
Liberty Centre is a specialist Centre for young adults and adults with learning disabilities including Autism and other related conditions with associated challenging behaviors. Our services include a specialist day Centre, outreach, and supported living Units. We offer a safe and friendly environment for our service users and employees.

Liberty Centre are currently looking to expand their fantastic team of Support Workers. As part of this role, you will be supporting individuals with learning disabilities and Autism.
* As a Support Worker, you will be responsible for providing excellent person-centered care, putting people at the heart of everything you do.
* Have good oral and written communication skills.
* You will communicate with family members, and other professionals who all work together to help individuals live independently and fulfilling lives within a safe and stimulating environment.
* Administer medication in line with medication policy and the service users’ support plans, where trained and authorized to do so.
* Support service users with regards to decision making and lifestyle choices enabling them to take control of their own lives, increasing independence.
* Support service users to take an active role in the community, (which has been risk assessed) by accessing local facilities and wider community resources in the spirit of social inclusion. Therefore, promoting the service users’ leisure interests and community participation.
* Support the development of person centred planning; ensuring each service user’s support plan has meaningful and achievable goals.
* To support service users’ through transition process when applicable.
* Work as a keyworker/ co keyworker to coordinate and develop the support to the service users’ including their Health Action Plans and Person Centred Plans.
* Contribute to / attend reviews and multi-disciplinary meetings where applicable.
* Work with service users’ to promote healthy lifestyles.
* Work in partnership with fellow colleagues, Service Coordinators, Service Managers and other professionals to maximise quality of life for service users.
* Develop positive relationships with service users’ family and carers.
* Work in line with Liberty’s Policy & Procedures in place, including the Safeguarding and whistle blowing policy to ensure that service users’ are kept safe at all times.

Don’t worry if you don’t have previous professional experience in care as we are looking for people with the right values and attitudes.
