Brain Injury Case Manager

Our client is seeking an experienced Brain Injury Case Manager to join their team working as an Associate. The ideal candidate will possess a strong qualified background in either Social Work, Mental Health Nursing, Occupational Therapy, or Psychology, with a focus on supporting clients with brain injuries and co-occurring challenges.
Offering a range of day-to-day case management services but also specialising in providing tailored assistance to individuals struggling with behavioural issues, and often other challenges including substance misuse issues, ensuring a holistic approach to recovery and rehabilitation.
This role requires a compassionate and proactive individual who can effectively advocate for clients and facilitate their journey following a personal injury or clinical negligence incident.
The position offers home working only with regular community visits to see clients.
What we need from you:
- You will need to hold a relevant degree in Social Work, Mental Health Nursing, Occupational Therapy, Psychology, or another related field
- Be registered with appropriate governing bodies such as IRCM when in place and or Social Work England, HCPC, NMWC
- Have proven experience in case management, particularly with children and adults, with disability, brain injury and substance misuse populations
- Strong understanding of safeguarding practices and experience working with vulnerable populations
- Have excellent communication and people skills, with the ability to build rapport with clients and their families
- Demonstrated ability to work independently and as part of a multidisciplinary team
- Strong organisational skills and attention to detail
- Ability to manage sensitive and confidential information with discretion
- Private home office space will also be required
Preferred Qualifications:
- Level 4 in Safeguarding or can demonstrate relevant experience
- Additional qualifications in supporting clients with trauma, challenging behaviours, and history of substance misuse
What the role involves:
- Conduct comprehensive assessments of clients needs, strengths, and challenges related to brain injuries and substance misuse
- You will develop and implement personalised case management plans in collaboration with clients, families, and multidisciplinary teams
- Provide ongoing support and guidance to clients and their families, ensuring they have access to necessary resources and services
- Provide regular Case Management reports that are a factual summary of clients' needs and can evidence SMART goals
- Monitor and evaluate client progress, adjusting case management plans as needed to reflect changing needs or circumstances
- Maintain accurate and up-to-date case records, documenting all interactions and interventions
- Collaborate with healthcare providers, social services, and community organisations to coordinate care and services
- Advocate for clients' rights and needs within healthcare and legal systems
- Ensure compliance with safeguarding policies and procedures, recognising and responding to signs of potential abuse or neglect.
**You are expected to keep your own training up to date and will need the appropriate Associate Insurances Public Liability and Professional Indemnity and a DBS in place**.
