Job DescriptionAs a Support Services Coordinator at Communitas, Inc, you will work one-on-one with supported individuals on community opportunities, life skills, self-care skills, and safety skills. Your goal is to help individuals reach their goals and work toward independence and positively impact their quality of life.
- Provide community and/or in-home direct services to individuals with various developmental disabilities between ages 5 to older adulthood.
- Work under supervision of individual's family and receive support of Communitas Family Support Coordinator.
- Valid driver's license, acceptable driving record and reliable transportation required.
