Job Overview
We are seeking a highly motivated and enthusiastic Health Visitor to join our Flying Start multi-agency team in Flintshire, North Wales. The successful candidate will manage a defined caseload, facilitating group and community work to address health inequalities in partnership with the wider team.
Main Duties
The post holder will undertake assessments of needs, planning, implementing, and evaluating the programme of care offered to families and individuals. They will work collaboratively with the multi-skilled team to achieve the Welsh Assembly Government initiative scheme, targeting communities in need to provide enhanced local services.
Requirements
Candidates must hold a degree or equivalent in Health Visiting (or be on a current SCPHN Health Visiting course) with current NMC registration and nurse prescribing qualification. Experience is preferable but may be newly qualified.
