Clinical Lead for Frailty/Home Treatment Service

About the Role
We are seeking an Advanced Clinical Practitioner to lead our East Kent HCP Frailty/Home Treatment Service. The successful candidate will be responsible for applying advanced clinical expertise to provide clinical care to older people in various healthcare settings.
Main Responsibilities
The Advanced Clinical Practitioner will lead and monitor improvements to quality of care through clinical audit, evidence-based practice, clinical supervision, teaching, and supporting professional colleagues. They will also be responsible for implementing relevant clinical guidelines into practice and contributing to the further development of the evidence base through research and audit.
Key Requirements
- Professional registration with NMC, HCPC, or GPhC
- Evidence of post-registration study
- Research process and governance knowledge
- Management of patients with long-term conditions
- Ability to undertake risk assessment
Desirable Skills
- Teaching qualification
- PGDip in Advanced Clinical Practice (or equivalent)
- Independent prescribing
- Autonomous practitioner
- Clinical examination skills
