Key Responsibilities:
- Work closely with the Nurse Manager to setup new care packages and maintain regular shifts within your client caseload.
- Uphold familiarity with each client's requirements, providing consistent and personalized care per their care plan.
- Identify and communicate upcoming staffing gaps and introduce new hires to clients.
- Mentor support workers and conduct observations to ensure they possess the necessary clinical skills.

Requirements:
- Previous experience working with people who have disabilities and complex health needs, such as tracheostomies, ventilators, gastrostomy tubes, and bowel management care.
- Attend mandatory paid training, including achieving a QCF Level 3 within two years of employment.
