Key responsibilities include acting as a first point of assistance for internal and external enquiries relating to patient discharge, facilitating and supporting effective communication between hospital staff, patients and their families, and managing patient communications via telephone and Trust systems.
The position is full-time, Monday to Friday 8am-4pm, with weekend working on a rota basis.
