About the Role

We are seeking a highly motivated and experienced Pharmacy Technician to join our team at East Cleveland PCN Ltd. As a Pharmacy Technician, you will play a key role in supporting clinically and cost-effective prescribing provision across our network population.

Key Responsibilities
  • Support the care management of patients with chronic diseases, undertaking clinical medication reviews to support proactive management of complex polypharmacy patients.
  • Develop and maintain clinically and cost-effective prescribing provision across the network population, improving patient outcomes and helping to relieve service and financial pressures.
  • Support the delivery of person-centered medicines optimisation, helping to standardise approaches to care across the PCN and forging strong relationships with specialist and community pharmacists.
  • Provide day-to-day support to practices in regards to prescription and medication queries, supporting the repeat prescription system, dealing with acute prescription requests, and medicines reconciliation on transfer of care.
Person Specification

Essential

  • BTEC in Pharmaceutical Sciences / QCF Level 3 in Pharmacy Services or equivalent.
  • Registered with the General Pharmaceutical Council.

Desirable

  • QCF Level 4 or above in a qualification relevant to the post.
  • Experience in conducting medication reviews.
  • Experience in supporting the implementation of new policies and processes to improve the quality and safety of practice, in the context of medicines optimization.
