Job Overview

We are seeking a Patient Flow Specialist Nurse to join our District Nurse Liaison Team in our efforts to provide high-quality care to patients with mental health and learning disabilities.

This role will involve working closely with the acute trust discharge team and Local Authority to ensure smooth and safe discharges for patients.

The successful candidate will have experience in supporting patients transfer from acute to community/home care and a strong understanding of the impact of DToCs on patients and the Organisation.

Main Duties and Responsibilities
  • Autonomously complete health needs assessments and continuing health care needs assessments to ensure the appropriate long-term care plan is in place.
  • Ensure excellent levels of customer service for all patients/relatives and staff, and demonstrate improvements in quality and patient experience.
  • Support all aspects of discharge paperwork and the planning and management of effective and timely patient discharge and transfer for patients with complex discharge needs.
  • Work in collaboration with operational partners across the health and social care system to develop, monitor, and expedite patient management plans for patients with complex needs.
Requirements and Qualifications
  • Evidence of post-basic continuing professional development.
  • Experience of supporting patients transfer from acute to community/home care.
  • Understanding the impact of DToCs on patients and the Organisation.
  • Ability to influence, negotiate, and enhance patient care.
  • High professional standards and understanding of professional issues.
  • Ability to offer professional guidance to staff.
