Mental Health Practitioner

This position involves working with patients who have severe mental illnesses (SMI), providing them with support and guidance to help them manage their conditions. The successful candidate will be responsible for assessing patients' needs, developing personalized care plans, and ensuring that they receive the necessary treatment and support.
The ideal candidate will have excellent clinical skills, strong communication and interpersonal skills, and the ability to work effectively in a fast-paced environment. They will also be required to maintain accurate records, complete health checks, and collaborate with other healthcare professionals to ensure seamless patient care.
Job Details
- Assessing and implementing evidence-based interventions for patients on the SMI register
- Optimizing community care and the least restrictive environment in line with recovery and social inclusion principles
- Collaborating with the SMI team and creating mental health plans for patients
- Liaising with specialist mental health services as needed
- Keeping accurate electronic records of patient care episodes
- Completing full health checks, including physical health checks and medication reviews
Salary: £37,500 - £42,000 per annum (dependent on experience)
