Care Act Specialist

The Midlands Partnership NHS Foundation Trust is seeking a highly skilled Care Act Specialist to join their team in Stafford. This full-time role involves working with adults over 18 years old, addressing their physical and mental health needs.
You will work closely with multi-disciplinary teams to promote independence and improve the health and wellbeing of the population served. As a key member of the Adult Social Care team, you will be responsible for completing holistic assessments to identify needs and provide support.
This dynamic role requires strong interpersonal skills, with the ability to work effectively with adults, carers, professionals, and voluntary agencies. You will also have access to ongoing training and support to ensure your continued professional development.
Key Responsibilities- Conduct comprehensive assessments to identify adult needs and develop effective support plans
- Work collaboratively with internal and external partners to address complex care issues
- Provide expert advice on social care and health services to ensure seamless delivery
