Job Overview
Age UK Doncaster is seeking a highly skilled individual to join their team as an Assessment Officer. This role involves supporting people living with dementia and their carers or families to access essential networks and information. The successful candidate will have excellent communication and organisational skills, experience in social care or health, and a passion for community work.

About the Role
The Assessment Officer will be responsible for carrying out initial holistic strengths-based assessments for people accessing Age UK Doncaster's services and projects. They will co-produce appropriate and proportionate support plans, supported signposting for individuals internally and to external support where necessary, and accurately record information and keep all systems up to date.

Key Responsibilities
- Conduct initial assessments and develop support plans
- Provide signposting and guidance to individuals and their carers
- Maintain accurate records and update systems accordingly
- Liaise with statutory agencies and maintain professional relationships

Requirements
The ideal candidate will have excellent interpersonal and communication skills, experience in social care or health, and a strong understanding of dementia and its impact on individuals and families. They will also have strong IT skills, including proficiency in Microsoft Office and experience using databases to manage information accurately.

Salary
The salary for this role is approximately £28,000-£32,000 per annum, depending on experience.
