Clinical Care Specialist

We are seeking a skilled Clinical Care Specialist to join our team at Healthcare Homes Group. As a Clinical Care Specialist, you will be responsible for providing high-quality clinical care to our residents, ensuring their physical and psychological needs are met.
Key Responsibilities:
* Provide clinical care to residents, including administering medicines and maintaining effective communication
* Support new admissions and discharges, maintaining confidentiality and adhering to regulatory frameworks
* Develop and implement detailed care plans, including risk assessments and diet plans
* Collaborate with care staff to deliver person-centred care that respects residents' rights and dignity
Requirements:
* Current NMC registration
* Good knowledge of clinical practices and regulatory frameworks
* Experience in writing detailed care plans and risk assessments
