The successful candidate will have a strong understanding of mental health principles and practices, with experience in working with older adults and individuals with complex needs. You will be responsible for assessing, coordinating care, and delivering interventions in the community, working closely with individuals, families, and other healthcare professionals.

Key Responsibilities:

Requirements:
