Job Title: Advanced Nurse Practitioner - Primary Care

We are seeking an experienced Advanced Nurse Practitioner to join our team at Crowhall Medical Group. As an Advanced Nurse Practitioner, you will play a vital role in providing high-quality patient care and contributing to the success of our Primary Care Network.

Key Responsibilities:
  1. Manage Complex Patient Cases: You will be responsible for managing patients presenting with a range of acute and chronic medical conditions, providing expert advice and guidance to patients and healthcare professionals.
  2. Prevent Long-term Conditions: Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects to their health.
  3. Provide Specialist Nursing Care: Provide advanced, specialist nursing care to patients as required, in accordance with clinical evidence, NICE, and the NSF.
  4. Collect and Analyze Data: Undertake the collection of pathological specimens, request pathology/radiology services as necessary, and interpret ECGs and process and interpret pathology and other test results.
  5. Deliver Patient-Centered Care: Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.
  6. Maintain Accurate Records: Maintain accurate clinical records in conjunction with extant legislation.
  7. Community Outreach: Carry out home visits to patients in our community, prioritizing health issues and intervening appropriately.
  8. Mental Health Support: Recognize, assess, and refer patients presenting with mental health needs.
  9. Medication Management: Support patients in the use of their prescribed medicines or over-the-counter medicines, reviewing annually as required.
  10. Safeguarding: Support the clinical team with all safeguarding matters in accordance with local and national policies.
  11. Substance Abuse: Understand practice and local policies for substance abuse and addictive behavior, referring patients appropriately.
  12. Health Promotion: Deliver opportunistic health promotion where appropriate, helping to deliver national immunization campaigns.

We utilize EMIS Web, Accurx, iPlato, and eConsult systems to ensure seamless patient care and communication.
