**Job Summary**

We are recruiting for an Advance Care Planning Coordinator to join our team. As a key member of our team, you will be responsible for promoting patient choice and autonomy by enabling individuals and families to make informed decisions about their care and treatment.

You will provide specialist information on advance care planning and support people to plan ahead and record their wishes for end-of-life treatment care. This is a new permanent position, with potential to develop within the role.

  • Work closely with individuals and families to promote patient choice and autonomy.
  • Develop and maintain relationships with healthcare professionals and other stakeholders to promote our service and support.

**Key Responsibilities**

The estimated salary for this role is £32,000 - £42,000 per annum. In addition to a competitive salary, you will also receive a range of benefits, including access to ongoing training and development opportunities.
