Job Overview
We are seeking an experienced nurse to take on a dual role, providing nursing support to our expert team of clinical staff on day shifts.
As a key member of our multidisciplinary team, you will work closely with the Senior Discharge Planners, Matrons, Lead Nurse, and Medical Team to manage the Discharge Planning & In-Reach service on the unit.
Key responsibilities include coordinating the In-reach Clinic, working in partnership with the multidisciplinary team to determine the best place for continuing care, and managing patient flow from admission through to discharge.
Main Duties
- Coordinate the In-reach Clinic to actively reduce length of stay.
- Work in partnership with the multidisciplinary team to determine the best place for continuing care.
- Manage patient flow from admission through to discharge, maintaining high quality patient care and efficient use of resources.
