About the Role

We are seeking a highly motivated and experienced paediatric nurse to join our specialist children's renal service. As a key member of our team, you will play a vital role in developing and advancing paediatric renal nursing practice across various care settings.

Main Responsibilities
  • Manage a caseload of children and young people from pre-dialysis to post-transplant, including transition to adult services.
  • Work closely with the paediatric renal team to promote, identify, assess, and manage care for potential paediatric transplant patients.
  • Act as a point of contact for families, multidisciplinary teams, other hospitals, and community agencies for specialist advice and information.
  • Support young people transitioning to adult services through the Transition Youth Program workshops.
  • Conduct home and school visits to support young people transitioning to adult services.

Requirements
  • First-level registration (Child)
  • Evidence of ongoing CPD
  • Demonstrable experience in relevant paediatric service
  • Significant post-registration experience
