This SEND Keyworker position is a key part of our team, supporting families and children in Tamworth. The ideal candidate will have excellent communication skills and be able to work independently and as part of a team.
Key Responsibilities:- COORDINATE WITH FAMILY PRACTITIONER LEADS TO DELIVER EDUCATION HEALTH & CARE ASSESSMENTS
- DEVELOP AND MAINTAIN EFFECTIVE RELATIONSHIPS WITH STAKEHOLDERS
- ENSURE PROVISION OF HIGH-QUALITY SUPPORT TO CHILDREN AND YOUNG PEOPLE WITH SPECIAL EDUCATIONAL NEEDS
- MONITOR AND EVALUATE THE EFFECTIVENESS OF SUPPORT PROVIDE
- Maintain accurate records and reports as required
