We are seeking a dedicated and experienced Care Leaver Pathway Manager to join our passionate and proactive team.

About the Role

As a key member of our team, you will be responsible for managing a team and ensuring that young people receive first-class pathway plans and timely support to meet their needs and identify/manage risks.

You will be a competent and dedicated manager who has insight and experience of the needs of care leavers, with a strong ability to lead and motivate your team.

Key Responsibilities
  • To manage a team and ensure that young people receive first-class pathway plans and timely support to meet their needs and identify/manage risks.
  • To work closely with colleagues and external partners to provide a coordinated approach to supporting care leavers.

Paid Salary: £38,000 - £42,000 per annum
