Care Manager with Rehabilitation Focus

We are looking for a skilled Care Manager to join our team at Liquid Personnel. The successful candidate will be responsible for conducting comprehensive assessments and developing rehabilitation plans for adults in need of social care intervention.
Main Responsibilities:
- Assess adults in need of social care, identifying their strengths and areas for improvement.
- Develop and implement rehabilitation plans, working closely with healthcare professionals and other stakeholders.
- Provide support and guidance to service users, helping them to achieve their goals and maintain their independence.
- Manage a caseload of service users, regularly reviewing and updating care plans as necessary.
Requirements:
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK licence
- Post-qualified experience
- Degree level or equivalent in Social Work
