Job Description

Job Title: Nurse - Care Specialist

Location: Lewin House, Aylesbury

Hours: Part-time – 12 hours per week

Pay: £23.48 per hour

We are seeking a skilled and compassionate Nurse to join our care team on a part-time basis at our nursing home in Aylesbury. As a Nurse - Care Specialist, you will play a vital role in delivering high-quality care and support to our residents, ensuring their physical, emotional, and social needs are met.

Main Responsibilities:

  • Provide respectful and person-centered care to residents, adhering to their individual care plans and promoting their independence and dignity.
  • Support the care team in achieving excellent care standards, participating in team meetings and contributing to the development of care plans.
  • Assist residents with daily living tasks, such as personal care, mobility, and end-of-life care, as directed in their care plans.
  • Administer medication as prescribed and recorded in the individual Medication Administration Record.
  • Ensure residents receive planned or urgent healthcare whenever required, collaborating with healthcare professionals as needed.
  • Record essential information in appropriate locations clearly, accurately, and immediately.
  • Actively promote the service and The Fremantle Trust by clear, respectful, and helpful communication with all stakeholders, including residents, relatives, healthcare professionals, and colleagues.
  • Contribute to the success of the care team by respecting colleagues and participating in team meetings and communications systems.

Requirements:

  • A valid NMC Pin
  • Evidence of clinical training
  • A minimum of 2 years previous experience working within a nursing home
  • Experience of care planning and key working principles
  • Understanding of relevant health & safety legislation
  • Working knowledge of the Regulatory Framework relating to the care of Older People
