Care Professional Wanted

We are seeking a committed Holistic Healthcare Assistant to join our Transformation Unit team.

**Expected Annual Salary:** £24,071 - £25,674

About the Position

This is an exciting opportunity to deliver holistic care to patients in their own homes as part of our Hospice at Home service.

Main Duties and Responsibilities
  1. Deliver high-quality care to patients in accordance with individualised care plans.
  2. Provide emotional support and practical assistance to patients and their families/carers.
  3. Maintain accurate records and contribute to a safe and respectful working environment.
  4. Work collaboratively with colleagues to achieve excellent patient outcomes.
  5. Recognise and respect the Equality and Diversity of patients and colleagues.
Required Skills and Qualifications
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Understanding of basic end of life symptoms and physiology
  • Proficiency in Electronic Patient Records (EPR)
Preferred Qualifications and Experience
  • Current UK driving license
  • Level 3 Care Certificate
  • Strong numerical and literacy skills
