At PP Associates Ltd, we are seeking an experienced Social Worker Level 3 to join our Adult Social Care team. Job Summary:

The successful candidate will be responsible for conducting comprehensive assessments of individuals needs, including physical, emotional, and social factors.

Key Responsibilities:

  • Conduct thorough assessments of individuals needs and develop person-centered care plans in collaboration with clients and their families.
  • Advocate for clients rights and ensure their voices are heard in the decision-making process.
  • Provide support and guidance to clients facing complex challenges, including mental health issues, disabilities, and social isolation.
  • Collaborate with multi-disciplinary teams, including health professionals, to ensure integrated service delivery.
  • Monitor and review care plans regularly, making adjustments as necessary to meet changing needs.
  • Maintain accurate and up-to-date case records in accordance with legal and organisational standards.
  • Participate in supervision, training, and professional development to enhance skills and knowledge.
  • Contribute to the development of policies and procedures related to adult social care services.

Qualifications:

  • A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Registration with Social Work England.
  • A minimum of X years experience working in adult social care.
  • Knowledge of relevant legislation, frameworks, and best practices in social work.

Estimated Salary: £45,000 - £55,000 per annum.
