Job Summary

We are seeking an experienced Practice Lead to join our team in Hull, working on adult social care projects. The successful candidate will be responsible for maintaining awareness of national and local contexts, facilitating case discussion and quality monitoring, and supporting joint working across adult social care.

Key Responsibilities

  • Maintain awareness of changes in national and local contexts and communicate effectively within and outside the organisation.
  • Facilitate case discussion and quality monitoring to ensure operating model principles are understood and implemented.
  • Support operational manager in nurturing joint working across adult social care, managing joint allocations and assessments.
  • Distribute guidance and support enabling people to make their own choices and decisions, applying and updating best practice into policy and practice.
  • Provide support, advice and encouragement to meet legislation, guidance policies and procedures.
  • Support operational manager in regular inspections/audits of premises and caseloads to ensure policies and systems focus on staff and people health and wellbeing.
  • Prepare evidence and write reports to support operational manager in dealing with complaints, workforce issues and court proceedings.

Salary

The salary for this role is up to £43,516 per annum, depending on experience.
