Job Summary

We are seeking a highly skilled and dedicated Registered Nurse to join our team in the Alton Primary Care Network. As a District Nurse, you will play a vital role in delivering high-quality patient care and working closely with our multidisciplinary team to ensure the best possible outcomes for our patients.

Key Responsibilities:

  • Deliver person-centred care to patients in their own homes, focusing on prevention, early intervention, and management of long-term conditions.
  • Work collaboratively with other healthcare professionals to develop and implement care plans that meet the individual needs of patients.
  • Conduct comprehensive assessments, identify health needs, and provide education and support to patients and their families.
  • Manage and coordinate care for patients with complex needs, including those with palliative care requirements.
  • Develop and maintain strong relationships with patients, families, and carers to ensure seamless care transitions and effective communication.

Requirements

To be successful in this role, you will need:

  • A current registration with the Nursing and Midwifery Council (NMC).
  • Experience of working in a community setting, preferably in a primary care network.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • A strong understanding of the principles of person-centred care and the ability to apply these in practice.
  • Ability to work autonomously and as part of a team, with a flexible approach to working hours and locations.
