About the Role

The Leader in Resident Care is a key position at our care home, where you will play a vital part in managing the care of our residents. We are looking for a Nurse Practitioner - Skilled Care who can provide high-quality and person-centred care, working collaboratively with multidisciplinary teams to assess, plan, implement, and evaluate care plans tailored to the individual needs of our residents.

Main Responsibilities

  • Deliver person centred care to each resident
  • Assess and monitor the physical health of residents within care plans
  • Support and supervise all care staff and new recruits
  • Ensure effective communication of any concerns relating to resident care
  • Comply with relevant regulatory and statutory requirements
  • Undertake any other duties that may be reasonably required as designated by the Home Manager
