Job Title: Adult Nurse Manager

We are seeking an experienced and passionate Adult Nurse Manager to join our team at Leaders in Care, a well-established provider of complex care packages within the community.

About the Role

The successful candidate will be responsible for providing face-to-face support to adult clients and their families, creating personalized care plans, and working closely with a core team of carers to deliver high-quality care.

Key Responsibilities
  • Design and deliver tailored care plans specific to individual client needs
  • Effectively manage the multidisciplinary team through strong communication and teamwork
  • Provide ongoing monitoring, support, and assessment of clients' physical and physiological needs
  • Deliver nursing interventions, including medicine management
  • Build and maintain relationships with clients' families
  • Work closely with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke packages of care
Requirements
  • RGN with an active NMC PIN
  • Supportive management style with experience of tracheostomy care and ventilators within the last 2 years
  • Advanced communication skills verbally and written to liaise effectively with patients, families, and the MDT
  • Suitable for Band 5 / Band 6 Nurses with evidence of effective leadership and management of HCAs
  • Full UK Driving Licence desirable but not essential
