Job Summary

We are seeking a skilled professional to join our team as a Placement Officer. The successful candidate will be responsible for sourcing domcare support packages to enable individuals to return home from hospital, where they have health and social care needs.

Key Responsibilities
  • To discharge the Authority's responsibilities under ASCOF and other relevant legislation with respect to Service Users with disabilities, vulnerabilities, and Eligible care needs.
  • To ensure the council's eligibility criteria, policies, and procedures are followed in relation to service users.
  • To create packages of support to prevent unnecessary reception of people into care and to assist individuals to remain in their own home where possible and/or to move to suitable and appropriate residential care.
  • To match service user needs to potential placements or providers.
Requirements
  • Experience of using computer packages and undertaking administrative tasks is essential.
  • Experience of Care Standards and relevant legislation and the CQC registration and approval process is essential.
  • Experience of negotiating agreements or contracts is essential.
  • Previous experience of a customer-focused environment is essential.
  • Evidence of continued professional and personal development is essential.
Essential Qualifications
  • Educated to a high level, A levels as a minimum is essential.
Working Arrangements

The role requires a current standard CRB/DBS clearance. The working hours are 22 hours per week, and the role will involve weekend working.
