About the Role

We are seeking an experienced and skilled Community Health Specialist to join our team at Epsom and St Helier University Hospitals NHS Trust. As a Proactive Care Matron, you will play a key role in delivering high-quality, patient-centered care to individuals with complex needs.

Key Responsibilities
  • Provide clinical triage and assessment for inbound referrals, assigning the appropriate urgency and determining the best clinician to undertake the visit from the multi-disciplinary team.
  • Support annual Learning Disability or Serious Mental Illness Health checks.
  • Care coordination, managing patient caseloads, and offering proactive support for frequent attenders, patients with cancer, and those at risk of cardiovascular disease.
  • Anticipatory care, liaising with primary, secondary, and community teams to reduce hospital admissions for patients with ambulatory care sensitive conditions.
  • Enhanced Health in Care Homes, proactive and targeted visits for specific cohorts of patients.
  • Support the coordination of all housebound annual vaccinations.
  • Manage a small team of non-clinical care coordinators to support the delivery of duties.
  • Demonstrate clinical expertise and act as a professional role model, working as part of an integrated team to develop services in line with patient needs.
Requirements
  • Professional Registration.
  • Previous experience in a community setting.
  • Understanding of Proactive Care Process.
  • Ability to work in a multidisciplinary team environment.
