About the Role
This exciting opportunity has arisen for a skilled and experienced Care Coordinator to join our team at Cera Care Ltd.
The successful candidate will be responsible for effectively managing the delivery of care services, ensuring that care workers are matched to clients based on their individual needs.
You will also be expected to work closely with colleagues to ensure seamless communication and collaboration.
Key Responsibilities
- Manage the delivery of care services to meet client needs and expectations
- Match care workers with clients based on their individual needs
- Work closely with colleagues to ensure seamless communication and collaboration
- Conduct regular care assessments and reviews to ensure continuity of care
- Monitor and report on client progress and any concerns or issues
- Develop and maintain relationships with clients, families, and care workers
Requirements
- NVQ/SVQ 2 or 3 qualification (not essential)
- Registered with the SSSC
- 2 years experience working as a care coordinator for a Domiciliary Care Agency
- Knowledge of CQC/CI/CIW Compliance
- Experience conducting care assessments, care reviews, and motivating care staff
- Organised, methodical, and attention to detail
- Flexibility around working schedules, roles, and responsibilities
- Ability to work as part of a team
- A driver with access to a vehicle

Salary
The salary for this position is £23,000 per annum, reflecting the level of experience and skills required.
