Role Overview

The successful candidate will be responsible for conducting assessments, developing care plans, and working collaboratively with multidisciplinary teams to ensure effective case management.

Key Responsibilities
  • Conduct statutory visits and assessments to identify needs and risks
  • Develop and implement care plans to safeguard children and promote positive outcomes
  • Work closely with social workers, therapists, and other professionals to ensure comprehensive support
  • Collaborate with parents, carers, and other stakeholders to ensure effective case management
Requirements
  • Post-qualifying experience in social care, preferably in a children's services setting
  • Registration with the relevant professional body (e.g., HCPC)
  • Excellent communication, interpersonal, and problem-solving skills
  • Ability to work independently and as part of a team
