Job Title: Adult Social Care Policy Lead

Midlands Partnership NHS Foundation Trust is seeking a highly skilled and experienced Adult Social Care Policy Lead to join our team. As a key member of our organization, you will play a vital role in shaping the future of adult social care services.

Key Responsibilities:
  1. Provide line management and 'day to day' support to Support Workers, ensuring they are equipped to deliver high-quality care to individuals requiring the Home First service.
  2. Support staff to deliver a high standard of care, as directed by their Support Plan, and ensure they have access to debriefing and support when dealing with emotionally challenging situations.
  3. Manage day-to-day staff attendance and performance, escalating issues through the relevant Trust policy as delegated by the Team Leader.
  4. Support the Team Leader in responding to service delivery issues, complaints, staffing issues, and shortages, resolving them quickly to minimize impact on service user experience.
  5. Undertake first visits to assess patients' environments, identifying risks, transcribing medication, and providing personal care as required.
  6. Assist in the management of a smooth and effective transition for service users exiting the in-house service provision.
  7. Foster effective relationships within the service user community, with external partners, and provide robust links between all community services.
  8. Ensure accurate record keeping is maintained in accordance with Trust policy.
  9. Ensure the service is delivered in line with Trust policies and procedures.
  10. Regularly monitor standards and the quality of service delivery through a suite of management information supported by internal and external audit.
  11. Assist in the collation of data/feedback of service user satisfaction and review of service performance.
  12. Assist with the Health and Safety Audit process and undertake risk assessments as delegated by the Operational Lead as required.
  13. Be flexible to work shifts and provide weekend cover as required by the service, including delivering personal care within the community.
  14. Ability to travel across the geographical footprint of the Trust to respond to the needs of the service.
  15. Support staff with raising concerns and reporting incidents as per Trust policies.
  16. Be aware of and ensure that staff adhere to Midlands Partnership Foundation Trust's Adult Safeguarding Enquiry policy and procedures.
  17. Comply with and ensure staff are aware of organisational policies, procedures, and legislative frameworks.
  18. Work within infection control guidelines to ensure that support given does not constitute a risk of infection either to the worker or the individual while working in the individual's home.
  19. Be aware and ensure that staff comply with all Trust policy requirements in relation to Information Governance, including the Freedom of Information Act 2000, the Data Protection Act, and the Trust's Electronic Record Management Standards & Guidance.
  20. Have an understanding of the Care Quality Standards (KLOE) to support the manager with the delivery of the service.
  21. Be flexible to undertake any other duties that may be allocated from time to time that is appropriate to the position and support the service needs.
  22. Communicate effectively with the wider team and be involved in decision-making by providing evidence to support any assessments required to determine potential needs and funding eligibility for long-term needs and further ongoing support.
  23. Answer and deal with general inquiries and telephone calls from internal and external stakeholders.
